Mare Breeding Passport
This Breeding Passport must be kept with the mare and each insemination and pregnancy check recorded. The mare must be checked by ultrasound 14- 18 days after the last insemination, at 30-35 days, and again at 50-60 days. After the final pregnancy check (50 –60 days after the last insemination) the Passport must be signed by the attending, licensed veterinarian and returned to the stallion owner (Watermark Farm). All information must be recorded and the Passport returned at the end of the breeding season whether the mare is in foal or not. The mare owner may wish to keep a copy. I, the undersigned veterinarian, duly licensed by the state of ____________, do hereby attest that on the above listed dates, I artificially inseminated the mare _______________ with semen from the stallion Adamant as indicated on the semen container and said inseminations were carried out in accordance with standard veterinary practices. Further, I hereby certify that the mare inseminated is the mare named herein and that no other mare was inseminated with the semen designated for this mare and that any excess semen was properly destroyed. Further, I certify that I am not an agent for the stallion owner and that I will indemnify and hold harmless the stallion owner from any claims arising from the negligent, improper or ineffective insemination by me. I also performed the above listed pregnancy examinations on the specified dates and attest that the recorded findings are accurate to the best of my knowledge.
